Tibialis Posterior Dysfunction

Overview

Tibialis posterior dysfunction (TPD) is a condition that affects the tibialis posterior tendon, which runs along the inside of the ankle and foot. This tendon plays a key role in supporting the arch and controlling foot movement during walking, running, and standing.


TPD occurs when the tendon becomes overused, inflamed, or degenerated, often due to repetitive activity, biomechanical issues, or age-related wear. Left unmanaged, it can lead to flattening of the arch (adult acquired flatfoot) and altered walking mechanics.


Common symptoms include:

  • Pain along the inside of the ankle and foot
  • Swelling or tenderness along the tendon
  • Flattening of the arch or changes in foot posture
  • Difficulty walking, running, or standing for long periods
  • Weakness when trying to stand on tiptoe

Physiotherapy Management

Physiotherapy management focuses on:

  • Reducing pain and inflammation
  • Restoring tendon strength and flexibility
  • Improving foot and ankle mechanics
  • Supporting the arch with exercises or orthotics if needed
  • Gradually returning to activity while protecting the tendon
